Adult Protective Services

We assist adults aged 60 or older and adults with disabilities aged 18 through 59 who are victims of abuse, neglect, and exploitation or who neglect themselves. Abuse includes any act by a caregiver or any other person who causes harm or serious risk of harm to a vulnerable adult.

Types of reportable abuse include: physical, emotional, sexual, passive neglect, deprivation, confinement, and financial exploitation. Self-neglect is an adult's inability, due to physical or mental impairments, or both, or a diminished capacity, to perform essential self-care tasks that substantially threaten his or her own health.


Making a Report of Suspected Abuse, Neglect, or Exploitation

Reports of suspected abuse can be made to:

  • Lifescape Community Services at 1-815-490-1125 during business hours.
    • Adult Protective Services Business hours are 8:30 AM to 5:00 PM Monday through Thursday and 8:30 AM to 4:30 PM on Fridays.
  • Illinois Department on Aging Adult Protective Services hotline at 1-866-800-1409. The hotline is available 24-hours a day.

 

All reports are kept confidential, and anonymous reports are accepted. The identity of the reporter is not disclosed without written permission from the reporter or by order of the court.

Our caseworkers are certified by the Illinois Department on Aging, undergo background checks and are dedicated professionals who provide personalized attention to their clients.

Participants must reside in Lee, Ogle, Carroll, or Whiteside County in northwestern Illinois.

Lifescape Community Services is the Illinois Department on Aging’s designated Adult Protective Services Agency for Whiteside, Lee, Carroll, and Ogle Counties.
