The Lifescape Adult Day Program provides a safe environment and enriching programming for functionally or cognitively impaired adults. The program strives to keep families together by meeting the needs of adults who want to remain in the least restricted environment, but need supervision during the day.
Lifescape Philosophy of Care
Each client is a unique individual with different strengths and weaknesses, has potential for growth and development, and has much to offer others. Our holistic approach addresses the physical, social, emotional and environmental aspects of our clients and their families. The program provides support for functional limitations and promotes positive attitudes that build capacity for independence. We strive to create a warm, respectful, home-like atmosphere that enhances involvement, enthusiasm, and enjoyment all while increasing stimulation of both mind and body.

- Transportation to/from center can be provided for a nominal fee.
- Services are provided by registered and licensed nurses and trained specialists with decades of caring experience.
- We maintain a client/staff ratio of six to one.
- Financial assistance may be available through a number of other community resources should you qualify, or private pay may also be an option. We have an admissions process that we are happy to help you navigate.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
