Our Services & Programs
Nutrition
(Winnebago, Boone, Lee and Ogle Counties)
Provides nutritious meals in a variety of settings for individuals ages 60+. Meals on Wheels delivers directly to the homes of eligible individuals and provides a daily "safety check" for those who are homebound. Individuals with more mobility can visit one of our many Senior Dining Sites in Winnebago, Boone, Lee or Ogle counties. Our Senior Dining Sites offer socialization, scheduled activities and educational presentations. Diners 60+ years of age can also visit one of our many restaurant-based congregate sites for a nutritious meal.
Adult Protective Services
(Whiteside, Lee, Carroll, and Ogle Counties)
Intervention for victims of abuse, neglect, & exploitation Adults age 60+ or 18+ with a disability. Reports of suspected abuse can be made during business hours to Lifescape at 815‐490‐1125, or any time to Illinois Department on Aging Adult Protective Services hotline at 866‐800‐1409.
Senior Choice
(Winnebago County)
Helps older adults choose lives of dignity and independence by providing assistance that includes, but is not limited to: individual appointments with caseworkers, transportation, paperwork, training and support to family members and caregivers. We provide confidential screening to identify all the benefits and services each individual may be eligible to receive. We may act as the person's advocate when needed. Our Grandparents Raising Grandchildren program aids in supporting both generations. Support groups provide help in understanding caregiving issues.
Adult Day Program
(Winnebago and Boone Counties)
Has more than 30 years of experience providing a safe and welcoming environment for participants. The newly opened Adult Day Program location in Rockford offers a variety of health, social and support services Monday through Friday. A registered nurse assesses each client and develops a program specific to the individual to support their well-being. Adult Day Program serves clients with cognitive disabilities including various dementias and Alzheimers. The program also serves the caregiver with support groups.
Health Promotion
(Winnebago, Boone, Ogle, Lee, Carroll, Whiteside, Stephenson, and Jo Daviess Counties)
Provides highest level, evidence based programs for seniors ages 60+. These programs promote positive life choices that improve one's health, wellness, and independence. These workshops are offered in a variety of settings, including: activity centers, places of worship, independent living communities, assisted living facilities, social service agencies, and more. We are always seeking host sites and participants for these programs. For more information on available workshops or to schedule a workshop, contact our Health & Wellness Coordinator.
Comprehensive Care Coordination
(Whiteside, Lee, Carroll, and Ogle Counties)
Complete assessment to coordinate the most appropriate care for each person's specific needs. Services are centered around case management and a plan of care, which may include: referrals & advocacy; home monitoring; obtaining an in‐home caregiver; obtaining in‐home emergency response systems.
Americorps Seniors Retired and Senior Volunteer Program (RSVP)
(Boone, Dekalb, Lee, Whiteside, and Winnebago Counties)
AmeriCorps Seniors (formerly known as RSVP) is one of the largest volunteer networks in the nation for people 55 and older. You can use the skills and talents you’ve learned over the years, or develop new ones while serving in a variety of volunteer activities within your community.
Senior Mental Health Program
(Winnebago County)
Our Senior Mental Health Program seeks to strengthen older adults' resilience and mental health well-being. We utilize validated screening tools to assist with recognizing Social Isolation, Loneliness, and Depression. Our Senior Mental Health Program implements the evidenced based program called Healthy IDEAS.
